‘I Wasn’t Able to Even Unseal My Eyes. That’s The Extent of the Swelling I Was’

Onemanda Preisinger is nervous about her child’s impending 13th birthday celebration. It’s not due to the usual reasons related to a home packed of clamorous preteen children, but because it’s the first time she will reveal her new face to acquaintances and relatives. “Clearly I’m going to inform all guests as they come in, ‘For your information, this is not how I appear,’” states the thirty-year-old real estate agent from south Florida.

Her appearance is, admittedly, a somewhat surprising – her face swollen and unnaturally tightened, as though secured by industrial-grade tape. Her new – and she’s eager to emphasize, short-term – look is the outcome of half a dozen cosmetic procedures, including an endoscopic mid-facelift, conducted by a surgeon in Turkey’s Istanbul, last month. “My spouse teared up when he viewed me for the first time because I couldn’t even open my eyes. That’s how puffy I was,” she tells me via online call from her home. “I had to tell him: ‘Honey, I’m okay, I’m not in pain. I just look like someone jumped me.’”

Increasing Popularity of Early Facelifts

Based on plastic surgeons, Preisinger is one of a growing number of people electing to undergo a rhytidectomy in their twenties and thirties – significantly before a ten years before typical surgeons’ typical patient age range of 40 to 60. (Though most surgeons are keen to emphasise the industry edict of: “We address genetics, not years.”) “I have individuals in their late twenties requesting facelifts,” states based in London cosmetic surgeon Georgios Orfaniotis, a ex-NHS doctor who focuses on the head and neck. “It’s a very significant surgery, and I’m a somewhat worried when I observe people opting for it as a lifestyle choice.”

Explaining the Rhytidectomy Surgery

A rhytidectomy, also known as a rhytidectomy, elevates the tissues in the face that start to sag as we age. “Our faces are structured a bit like onion layers,” explains Kent-based face specialist Marc Pacifico. “Skin and fat on the surface, then a layer of supportive tissue called the superficial musculoaponeurotic system. Think of the facial framework as the structural foundation of the face. And that is what causes aging; that’s what loosens and sags and pulls the dermis down with it.”

You risk performing surgery on individuals that have deeper issues. It’s not a practice for an conscientious surgeon to pursue

Previously reserved of affluent seniors, overuse of injectables – when excessive use of injectable gels stretches the face and leads to looseness in the dermis – combined with the common adoption of slimming medications, low-cost surgical travel, and the advancement of new, celebrity-endorsed surgical techniques are driving a new kind of customer towards this major operation. Reports show an eight percent rise in facial lifts over the last year in the United Kingdom; while a survey revealed that the portion of younger facelift patients is increasing, with one-third of procedures now conducted for individuals aged 35-55.

“Patients are asking now to appear as the optimal form of themselves and naturally the methods are following,” comments Orfaniotis. Currently, the operation du jour is the comprehensive facial lift, a technique evangelised by figures including Kardashian matriarch Kris Jenner, to style creator Marc Jacobs.

Deep Plane Technique

While a conventional facelift involves elevating the superficial layer, the comprehensive lift releases the underlying structures beneath it, enabling doctors to restructure the face by moving the deeper tissue, and avoiding the tight, stretched appearance occasionally caused by older methods. “We avoid placing tension on the dermis because we’re targeting deeper, to the deeper tissue,” explains prestigious London cosmetic doctor Rajiv Grover, who specialises in the deep plane technique. “It is the underlying layers that is repositioned. The skin follows along as a passenger.” Elevating the entire soft tissue layer as a unified block allows for a more natural-looking and longer-lasting result. It also means that the surgery is not just being used for the express purpose of anti-ageing – or revitalization, as the aesthetics industry terms it – but for “beautification” and facial restructuring, too. “We’re getting many inquiries from patients aged late twenties to mid-thirties for this purpose,” notes Grover.

Personal Journey

Preisinger had not intended on getting a facial lift – at minimum not at the thirty years old. But years of using injectable gels in an effort at “symmetry correction” meant that by the time she reached her late twenties, she was “damaged”. “I didn’t do this because I wanted to look young,” she clarifies. “I did it because the filler ruined my face. I wish I had never done it. If I had avoided the filler, I don’t think I would be where I’m at currently.”

If dermal fillers ever fully dissolve has historically been a debated topic in the aesthetics industry. “Research have shown that not only do they rarely completely dissolve,” says Grover, “but they migrate and can obstruct drainage pathways. A contributing factor to what we call ‘puffy appearance’ is also the reality that to some extent, the face becomes somewhat waterlogged because its capacity to remove lymphatic fluid has been diminished.” Though research into the area is early-stage, warnings on dermal fillers’ potential impact on the lymphatic system have been issued, and further research initiated. An esteemed face specialist, speaking on the condition of anonymity, mentioned he would avoid using fillers in a patient because of the dangers they present. “A lot of surgeons avoid talk about this, because the manufacturers who make filler can be quite forceful.” He’s heard stories, he adds, of cosmetic doctors facing court orders after expressing worries.

Decision and Procedure

For Preisinger, once she started using injectables, she believed she had to keep replenishing it – particularly when it began moving to different areas of her face. She was just 28 when a local plastic surgeon in Florida suggested that a facial and neck surgery might be what she required to exit the cycle of treatments. Two years later, she ended up selecting from a selection of suggested accommodations in Istanbul, texted to her by the patient coordinator at her doctor’s clinic.

Initially, Preisinger had scheduled an upper and lower blepharoplasty – better known as eyelid surgery – but her doctor advised her that a facial lift was the correct option for the laxity she was experiencing in her face. She arranged a forehead lift, a cheek lift and a jawline surgery. The day before her surgery, 24 hours post she’d arrived solo in Turkey, she opted to include a lip lift. “Subsequently the surgeon said, ‘We’re going to lift the under-eye bags.’ Then he said, ‘I believe if we extract some cheek fat, it will contour your face.’ Thus I ultimately including additional procedures,” she recalls. She spent $22,000 (sixteen thousand five hundred pounds) for the half-day operation, plus a three-day|
